Van Tattoo Waiver, Release & Consent FOrm

This agreement is made between the below party and Van Tattoo ( 1012 W Broadway, Vancouver ) .

Please read all the information below before you fill out this waiver form.

1. Consent and Legal Age
1.1. The Client confirms they are at least 18 years of age and legally capable of entering into this agreement.
1.2. The Client acknowledges that tattoos are a permanent body modification and that removal is difficult and costly.

2. Health and Medical Information
2.1. The Client confirms they have disclosed any relevant medical conditions, allergies, or sensitivities that could affect the tattoo process, including but not limited to:
• Bloodborne illnesses (e.g., Hepatitis, HIV)
• Skin conditions (e.g., eczema, psoriasis)
• Allergies to tattoo ink, latex, or cleaning products

2.2. The Studio reserves the right to refuse service if health or safety concerns arise.

3. Tattoo Design and Approval
3.1. The Client confirms their approval of the tattoo design, placement, and size before the procedure begins.
3.2. Any changes to the design after the tattoo session begins may result in additional charges.

4. Aftercare Instructions
4.1. The Studio will provide detailed aftercare instructions.
4.2. The Client agrees to follow these instructions to ensure proper healing of the tattoo.
4.3. The Studio is not responsible for issues arising from failure to follow aftercare guidelines.

5. Waiver of Liability
5.1. The Client understands and accepts the inherent risks associated with tattooing, including but not limited to:
• Minor bleeding, swelling, or discomfort
• Risk of infection or allergic reactions
• Scarring or fading over time

5.2. The Studio and its artists are not liable for any reactions or complications that may arise after the tattoo procedure.

6. Studio Rules
6.1. The Client agrees to respect the Studio’s policies, including:
• No food or drinks in the tattooing area
• No children or pets allowed during the session

6.2. The Studio reserves the right to refuse service to anyone exhibiting inappropriate or disruptive behavior.

7. Copyright and Photographs
7.1. The Client grants the Studio permission to photograph their tattoo for promotional purposes (e.g., social media, portfolio), unless otherwise stated in writing.
7.2. The tattoo design remains the intellectual property of the artist and Studio unless agreed otherwise in writing.

Acknowledgment and Signature
By signing below, the Client acknowledges they have read, understood, and agreed to the terms outlined in this Agreement.
